ABSTRACT

【Objective】 To analyze the efficacy and safety of intralesional triamcinolone acetonide in the treatment of mass granulomatous mastitis (GLM). 【Methods】 Retrospective analysis was made on 67 patients with GLM who were treated in Xijing Hospital from July 2021 to May 2022 and met the inclusion criteria. Among them, 31 patients were treated with local injection of triamcinolone acetonide, while 36 ones were treated with oral methylprednisolone. All the included patients underwent surgical treatment after their condition met the surgical criteria, and the follow-up period lasted for up to six months after surgery. The two groups were compared in improvement of symptoms and signs, treatment time, clinical effectiveness and safety, and recurrence rate. 【Results】 The effective rate of triamcinolone acetonide group and methylprednisolone group was 100%, but the clinical cure rate was significantly higher in triamcinolone acetonide group than in methylprednisolone group in stratified analysis (P<0.05). The improvement time of symptoms and signs in triamcinolone acetonide group was significantly shorter than that in methylprednisolone group (P<0.05). The incidence of side effects in triamcinolone acetonide group was also significantly reduced compared to methylprednisolone group (P<0.05). Follow-up for half a year showed no recurrence in both groups. 【Conclusion】 Compared with the traditional oral methylprednisolone group, local injection of triamcinolone acetonide in the treatment of mass granulomatous mastitis can rapidly relieve clinical symptoms and signs, shorten treatment time, and has higher efficiency and fewer side effects. Local injection of hormone combined with surgery is effective in treating mass granulomatous mastitis with low recurrence rate.

ABSTRACT

Objective:To explore the value of red blood cell distribution width to platelet ratio (RPR), C-reactive protein to albumin ratio (CAR) combined with bedside index for severity in acute pancreatitis (BISAP) score in assessing the severity of acute pancreatitis (AP).Methods:The AP patients in the First Affiliated Hospital of Jinzhou Medical University from January to December 2020 were respectively collected. According to the severity of the disease, the patients were divided into the mild acute pancreatitis (MAP) group, moderate severe acute pancreatitis (MSAP) group, and severe acute pancreatitis (SAP) group. The general information and laboratory indicators of the patients were collected and scored according to the BISAP scoring standard. Spearman correlation analysis was used to explore the correlation of RPR, CAR and BISAP score in three groups of patients and their correlation with AP severity. Model 1 [MAP group and non-MAP group (MSAP group + SAP group)] and model 2 [non-SAP group (MAP group + MSAP group) and SAP group] were constructed. Multivariate binary logistic regression was used to analyze the independent factors of the non-MAP group and SAP group. The receiver operating characteristic (ROC) curve was used to calculate the area under the curve (AUC) to analyze the value of RPR, CAR, and BISAP score alone and in combination to judge the severity of AP patients.Results:A total of 197 AP patients who met the criteria were included, including 102 MAP patients, 56 MSAP patients, and 39 SAP patients. There were significant differences in RPR, CAR and BISAP score among patients with different AP severity ( P<0.001). RPR, CAR and BISAP score were positively correlated, and all three were positively correlated with AP severity ( r=0.435, 0.490, 0.628, P<0.001). RPR and CAR were independent factors for the severity of AP, and the combination of RPR, CAR and BISAP score was better than a single indicator in judging the severity of AP patients. The AUC of the three combined in Model 1 and Model 2 were 0.868 and 0.889, respectively. Conclusions:RPR, CAR combined with BISAP score has a good application value in the evaluation of AP, and is suitable for clinical promotion.

ABSTRACT

Objective:To observe any effect of combining ultrasound-guided stellate ganglion pulsed radiofrequency irradiation with electroacupuncture in treating migraine.Methods:Seventy-two migraine patients were randomly divided into a combined group, an electroacupuncture group and a control group, each of 24. All three groups were treated with etocoxib and zolmitriptan dispersible tablets, while the electroacupuncture group and the combined group were additionally provided with electroacupuncture or ultrasound-guided stellate ganglion pulsed radiofrequency irradiation combined with electroacupuncture. The therapeutic interventions were administered once daily for 7 days. A visual analogue scale (VAS), a migraine-specific quality of life questionnaire (MSQOL), the Hamilton anxiety scale (HAMA), the Hamilton depression scale (HAMD) and a migraine disability scale (MIDAS) were used to evaluate each patient before the experiment and on the 3rd, 7th, 30th and 90th days after completion of the treatment.Results:The average VAS, MSQOL, HAMA and HAMD scores of the combined and acupuncture groups had improved significantly at all time points. Moreover, the average MIDAS scores of the combined and acupuncture groups had improved significantly 30 and 90 days after the treatment. On the 3rd day after the treatment, the average VAS, MSQoL, HAMA and HAMD scores of the combined group were significantly higher than the acupuncture group′s averages, while the average MIDAS score of the former group was significantly lower on the 30th and 90th days after the treatment. The average VAS, HAMA and HAMD scores of the combined group were significantly lower than the control group′s averages 3, 7, 30 and 90 days after the treatment, while their average MSQOL score was significantly higher. The average MIDAS score of the combined group was significantly lower than the control group′s average 30 and 90 days after the treatment.Conclusion:Ultrasound-guided stellate ganglion pulsed radiofrequency irradiation combined with electroacupuncture can significantly relieve the symptoms of migraine and improve the life quality of migraine patients for at least 3 months.

ABSTRACT

This paper analyzed the feasibility of SPOC-mixed teaching in view of bottlenecks faced by the current health statistics curriculum, and deeply analyzed the limitations of traditional classroom teaching and network teaching only, as well as the advantages of SPOC-mixed teaching mode. At the same time, the construction of SPOC-mixed teaching mode of health statistics curriculum was explored from three aspects: teaching preparation, teaching implementation and teaching evaluation. It is hoped that the traditional teaching mode of "mainly teaching' "existed in health statistics will be transformed into a student-led and learning-based mode. According to each student's learning levels, professional background and cognitive style, individualized teaching was conducted to teach students in accordance with their ability, and promote the reform on traditional education concepts and teaching modes of health statistics.

ABSTRACT

Objective To study the clinical use of microwave scalpel in hepatectomy for hepatocellular carcinoma without hepatic vascular inflow occlusion.Methods A retrospective study was conducted on the clinical data of 126 consecutive patients with hepatocellular carcinoma who were treated at the Department of Hepatobiliary Surgery of the Third Affiliated Hospital of Sun Yat-sen University from December 2015 to August 2018.The patients included 111 males and 15 females,with ages which ranged from 25 to 75 years.These patients were divided into two groups by the different surgical resection methods:the microwave scalpel hepatectomy (MSH) group which employed microwave scalpels to perform hepatectomy (n =42),and the ultrasound scalpel hepatectomy (USH) group which employed ultrasound scalpels (n =84).The perioperative conditions which included baseline data,amount of intraoperative bleeding,liver function after operation,and morbidity of postoperative complications were compared.Results There were no significant differences in the preoperative indexes which included tumor diameter,number of tumors,levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) between the 2 groups (all P > 0.05).In the MSH group,the amount of intraoperative bleeding was 100.0 (100.0,200.0) ml,which was significantly lower than the 300.0 (100.0,400.0) ml in the USH group (P < 0.05).Compared with the USH group,the levels of AST and ALT on postoperative day 1,3,7 after operation in the MSH group were significantly lower (all P < 0.05),and the corresponding albumin levels were significantly higher than the USH group (P <0.05).The incidence of complications was 4.8% (2/42) in the MSH group which was significantly lower than that of 20.2% (17/84) in the USH group (P < 0.05).Conclusion Microwave scalpel significantly reduced intraoperative bleeding and postoperative complications,and led to less liver functional injury.

ABSTRACT

Objective@#To evaluate the feasibility and safety percutaneous pulmonary vein intervention in patients with severe pulmonary vein stenosis (PVS) caused by fibrosing mediastinitis(FM).@*Methods@#This retrospective analysis included 5 FM patients (2 male, 3 female, 54-77 years old) confirmed by clinical presentation and chest computed tomography (CT) scan from January to June 2018 who were from Gansu Provincial Hospital and Shanghai Chest Hospital. CT pulmonary angiography (CTPA) further revealed severe PVS caused by fibrotic tissue compression in mediastinum. After selective pulmonary vein angiography, gradually balloon angioplasty was used to expand the pulmonary vein and then stents were implanted in the pre-dilated stenotic pulmonary veins. Evaluation of therapeutic effect was made at 6 months after the procedure.@*Results@#All of 11 serious compression PVS were treated with stent implantation (diameter: 7-10 mm, length: 17-27 mm). After stenting, degree of pulmonary vein stenosis decreased from (83±16)% to (12±4)% (P<0.01). The minimal diameter of the stenotic pulmonary vein was significantly increased from (0.8±0.5)mm to (7.5±0.8)mm (P<0.01). Trans-stenotic gradient decreased from (27.0±15.1)mmHg (1 mmHg=0.133 kPa) to (2.50±0.58)mmHg (P<0.05). Mean pulmonary pressure measured by cardiac catheter decreased from (45.0±9.0)mmHg to (38.7±8.4)mmHg (P<0.05). One patient experienced cardiac arrest due to vagal nerve reflex during big sizing balloon stent dilation and recovered after cardiopulmonary resuscitation. There were no other serious procedure related complications. During the follow-up, severe stenosis at end of proximal stent was evidenced in 1 patient due to fibrotic compression, and another patient developed in-stent thrombosis due to discontinuation of prescribed anticoagulant.@*Conclusion@#Percutaneous intervention for severe pulmonary vein stenosis caused by FM is feasible and safe, and can improve hemodynamic caused by the compression of mediastinal vascular structures in these carefully selected patients.

ABSTRACT

Objective To investigate the relationship between muscle mass reduction and quality of life in elderly lung cancer patients at the initial visit.Methods Totally 221 patients with non-small cell lung cancer undergoing chest and abdominal CT examination were included in this study at Fenyang Hospital in Shanxi Province.The Pearson's correlations of skeletal muscle index(SMI) and skeletal muscle radiodensity(SMD)with the disease stage,physical function,role of physical function,global quality of life,fatigue and dyspnea were analyzed.Results The average age of 221 patients was(70.2t9.0)years.The mean SMI for men and women was(46.7± 7.8)cm2/m2 and(39.4 ±6.4) cm2/m2 respectively.Multivariate linear regression analysis showed that SMI affected the body function,role of physical function,global quality of life,and fatigue dimension(all P < 0.05),and SMD affected the physical function of life quality and dyspnea dimension(P <0.05).SMI was related to the physical function and role of physical function(men:r =0.287 and 0.245,P =0.001 and 0.006;women:r=0.296 and 0.395,P =0.004 and 0.000).In male patients,SMO was also associated with low global quality of life and fatigue(r=0.191 and-0.198,P =0.032 and 0.026).The above relationships did not change even after adjusting for the effect of age and disease stages.Low SMD was associated with low physical function and dyspnea in both men and women(men:r =0.287 and 0.245,P=0.037 and 0.049;women.:r=0.281 and-0.262,P=0.006 and 0.010),and SMD was associated with dyspnea only in women even after adjusting for age and disease stages.A correlation between SMI and the disease stage was found in both men and women(male:r =-0.266,P=0.003;female:r=-0.223,P=0.005).Conclusions Changes in muscle mass may affect the physical function,role of physical function and global quality of life in elderly patients with non-small cell lung cancer.

ABSTRACT

Objective To observe the clinical effect of pulsed radiofrequency electrotherapy combined with acupuncture or electroacupuncture in treating neuralgia after herpes zoster infection.Methods Eighty-eight patients were divided into a control group,an electroacupuncture group,a pulsed electrotherapy group and a combined group using a random number table.The 4 groups were routinely given gabapentin and tramadol,while the electroacupuncture group was additionally treated with electroacupuncture,the pulsed electrotherapy group received radiofrequency irradiation of the dorsal root ganglion,and the combined group was given CT-guided pulsed radiofrequency electrotherapy combined with electroacupuncture or acupuncture.Before the treatment and 15 days and 1 month afterward,everyone was evaluated using a visual analogue scale (VAS) for pain intensity,the Hamilton anxiety (HAMA) and depression (HAMD) scales and the short form 36 (QOL-SF36) quality of life assessment.The area of painful skin was also measured.Results There were no significant differences among the 4 groups at the outset in any of the measurements.Significant improvement was observed in all of the measurements 15 and 30 days after the treatment except in the control group.The combined group was by then significantly better than the other 3 groups.Their dosage of gabapentin and tramadol had decreased significantly compared with the other groups after 3 months of treatment.Conclusion CT-guided pulsed radiofrequency electrotherapy combined with acupuncture or electroacupuncture is safe and effective for treating post-herpes neuralgia.It is worthy of clinical application and promotion.

Objective To investigate the current status of posttraumatic growth among digestive system cancer patient' families, and explore correlation of posttraumatic growth with social support, coping style in digestive system cancer patient' s families. Methods The Posttraumatic Growth Inventory, Perceived Social Support Scale and Simplified Coping Style Questionnaire were used to survey the 280 digestive system cancer patient' s families. Results The mean posttraumatic growth total score in families of the patients with digestive system cancer was (46.03 ± 15.83) points. The highest score was found for appreciation of life, the lowest was spiritual change. Meanwhile, posttraumatic growth was significantly correlated with perceived social support. The correlation coefficient was 0.730 (P<0.01). In the coping style, there was a positive correlation between positive coping style and post-traumatic growth. The correlation coefficient was 0.644, (P<0.01). While except personal relationship and mental change dimensions, there was a negative correlation between the post-traumatic growth with the negative coping style. The correlation coefficient was-0.155 (P<0.05). Conclusions Family of patients with digestive system cancer also experience posttraumatic growth when their loved ones are diagnosed with cancer, but the posttraumatic growth was at the lower level. Social support and positive coping are the positive factors of post traumatic growth, and negative coping is the negative influencing factor.

ABSTRACT

Objective To improve the in vivo analgesic activity of acacetin and find leads for the development of new drugs, novel acacetin derivatives containing alkyl amide groups with different length of carbon chain were designed and synthesized according to the molecular structure of the active hit compound found in our previous work. Methods Using apigenin as the initial chemical ma-terial,the acacetin was synthesized through 3 steps,then the target compounds were prepared by conjugating hydroxy group of acace-tin at position 7 with bromoalkyl amides. The analgesic activity of the target compounds was evaluated by acetic acid writhing model of mice. Results and Conclusion Novel acacetin alkyl amide derivatives showed more potent analgesic activities than that of clinical medicine diclofenac,which could be used as leads for further development of new drugs.

ABSTRACT

Clinical data of granulomatosa cheilitis of 33 cases were analysed.Odontogenic infectious foci were found in each patient.Elimination of odontogenic infectious foci was followed by regression or disappearance of the swellings in all the 33 cases,suggesting a close relationship between granulomatous cheilitis and odontogenic infectious foci.Elimination of odontogenic infectious foci can be an important measure for the treatment of granulomatous cheilitis.

ABSTRACT

To compare therapeutic effects of Five Needles of the Nape and routine acupuncture on treatment of pseudo bulbar paralysis dysphagia after Brain stroke. 60 patients were randomly divided into five key groups and routine acupuncture, and 30 cases in each group. The group of Five Needles of the Nape set points to take dumb door, Tianzhu, cure choke point with acupuncture treatment which cooperate with swallowing training. The group of routine acupuncture set points to take Lian Quan, Tong Li, Zhao Hai points with acupuncture treatment. Both groups were acupunctured once a day, 6 times a week and 2 weeks is a period of treatment, evaluating curative effect after two courses of treatment. The total effective rate of Five Needles of the Nape group was 93.3%, when the total effective rate of routine acupuncture group was 80.0%.as a consequence Five Needles of the Nape group is superior to routine acupuncture group [P<0.05]. Five Needles of the Nape have better clinical efficacy than routine acupuncture on treatment of pseudo bulbar paralysis dysphagia after Brain stroke

ABSTRACT

To observe influences of Bushen Xingnao Decoction [BSXND] on expression of vascular endothelial growth factor [VEGF], IL-1beta and tumor necrosis factor-alpha [TNF-alpha] in brain tissues and serum level of vascular dementia rats and to investigate neuroprotective mechanism of BSXND for vascular dementia. Wistar rats were randomly divided into normal group [N group], sham operation group [S group], dementia model group [M group] and Bushen Xingnao decoction treatment group [MT group]. After the model was successfully established, 2, 4, 6 weeks were regarded as observation point. Expressions of VEGF, IL-1beta and TNF-alpha in serum and brain of rat brain were measured by enzyme linked immunosorbent assay. The expressions of IL-1beta and TNF-alpha in MT group were lower than those in M group [P< 0.05], the expression of VEGF in MT group was higher than that in M group [P< 0.05]; the expressions of VEGF, IL-1beta and TNF-alpha in MT and M groups were higher than those in N and S groups [P<0.01]. BSXND can reduce the levels of IL-1beta and TNF-alpha in brain tissues and serum of vascular dementia rats and increase the expression of VEGF. BSXND can play cerebral protective role by suppressing the neuroinflammation response of vascular dementia rats and enhancing vascular repair

ABSTRACT

<p><b>OBJECTIVE</b>To dynamically analyze the change of xerostomia in patients with nasopharyngeal carcinoma after radiotherapy by DW MRI.</p><p><b>METHODS</b>Twenty-three nasopharyngeal carcinoma patients confirmed by pathology were enrolled. Male/Female: 19/4. The age was from 37 to 69 years. The patients were divided into two groups: G1, Dmean<26 Gy, G2, Dmean ≥ 26 Gy. All patients underwent salivary glands examination by DW MRI before IMRT, at the end of IMRT, 6 months and 12 months after IMRT, at the same time the ADC value of salivary glands were calculated. According to the RTOG/EORTC salivary gland injury grading standard and referring the subjective index, the degree of xerostomia was assessed. SPSS 13.0 and SAS 8.2 software were used to analyze the data.</p><p><b>RESULTS</b>At the end of IMRT, the change tendency of ADC in parotid and submandibular glands value was different in patients with different degree of xerostomia (F = 11.52, P < 0.01). At the end of IMRT, a significant difference for degree of xerostomia could be found in patients within different irradiation dose groups (Z = -3.622, P < 0.01). Clinical stage, treatment mode and age had no significant effect on the degree of xerostomia for patients at the end of IMRT (Z value was -0.791, -0.949, 2.488, all P > 0.05). A significant difference of xerostomia degree in patients was found at the various follow-up time after IMRT (χ(2) = 19.59, P < 0.01).</p><p><b>CONCLUSIONS</b>There is good correlation between the function of salivary gland and subjective rating of xerostomia in patients with nasopharyngeal carcinoma after radiotherapy. The degrees of salivary gland function and dry mouth in patients with nasopharyngeal carcinoma damage evaluate with illuminated dose increases. The function of salivary gland gradually restored and the degree of dry mouth gradually reduce with the extension of time after radiotherapy.</p>

<p><b>OBJECTIVE</b>To evaluate the dynamic change of salivary gland function in patients with nasopharyngeal carcinoma (NPC) before and after intensity modulated radiotherapy (IMRT) by DWMRI.</p><p><b>METHODS</b>Twenty-three NPC patients confirmed by pathology were enrolled. Male/Female: 19/4. The age was from 37 to 69 years. All cases underwent salivary glands examination by DWMRI before IMRT, at the end of IMRT, 6 months and 12 months after IMRT, at the same time the ADC value of salivary glands were calculated. The patients were divided into two groups: G1, Dmean < 26 Gy, G2, Dmean ≥ 26 Gy. SPSS 13.0 and SAS 8.2 software was used to analyze the date.</p><p><b>RESULTS</b>The ADC values of parotid in two group patients at rest were not significantly different between 6 months after and at the end of IMRT (t value was 0.732, 1.405, all P > 0.05). The ADC values of parotid in two group patients at rest were significantly lower in the 12 months after IMRT than at the end of IMRT (t value 2.621, 3.417, all P < 0.05). During the period of acid stimulation, A significant change of ADC in parotid was found in G1 (all P < 0.05) and no significant change of ADC in parotid was found in G2 at the various follow-up time after IMRT (all P > 0.05). At the various follow-up time after IMRT, the average ADC values of parotid were significantly higher in G2 than G1 in the same time points of acid stimulation (all P < 0.05). The ADC values of submandibular glands at rest were significantly higher at the various follow-up time after IMRT than before IMRT (t value was -9.782, -8.963, -7.722, all P < 0.05). During the period of acid stimulation no significant change of ADC in submandibular glands was found after IMRT (F = 1.124, P = -0.348).</p><p><b>CONCLUSIONS</b>The changes of salivary function can be well reflected by ADC values. There is a good clinical application value of DWMRI for evaluating radiation-induced salivary glands functional changes and observing salivary function recovery during the follow-up period after IMRT.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the expression and significance of miRNAs and Th17 related cytokines in patients with multiple myeloma (MM).</p><p><b>METHODS</b>A total of 27 MM patients and 8 health controls were enrolled in this study. The expression of miR-15a/16,miR-34a,miR-194-2-192 cluster and miR-181a/b in bone marrow were detected by real-time quantitative PCR (qRT-PCR). Enzyme-linked immunosorbent assay (ELISA) was used to determine the levels of Th17 related cytokines interleukin-17 (IL-17), IL-21, IL-22, IL-23 and IL-27 in peripheral blood plasma. The role of miRNAs and Th17 related cytokines was analyzed in the development of MM.</p><p><b>RESULTS</b>The expression of miR-15a/16,miR-34a,miR-194-2-192 cluster in MM patients were significantly lower than those of the health controls, while miR-181a/b were exactly the reverse (P<0.05). The levels of IL-17, IL-21 and IL-27 were up-regulated in MM patients compared to health controls while IL-22 was down-regulated (P<0.05). There was no significant difference of IL-23 between the two groups. The levels of miRNAs and Th17 related cytokines had associated with ISS but not with some clinical parameters (such as gender, age, disease classification). Higher expression of IL-17, IL-21, IL-23, IL-27, miR-181a/b and lower expression of miR-15a/16,miR-34a,miR-194 and IL-22 were observed in the end stage than the early stage of MM patients (P<0.05). There was a significant correlation between miRNAs and Th17 related cytokines.</p><p><b>CONCLUSION</b>Up-regulated IL-17, IL-21 and IL-27 may potentially down-regulate the expression of several miRNAs in MM patients. Establishment of the relationship may be useful for understanding the pathogenesis of MM and for clinical diagnosis of the disease.</p>

ABSTRACT

Objective To explore the optimal timing of liver transplantation for acute liver failure,and analyze relative risk factors affecting patients' prognosis.Methods We retrospectively analyze perioperative clinical data of 50 patients suffering from acute liver failure who underwent liver transplantation between March 2005 and June 2010 in Organ Transplantation Center,the First Affiliated Hospital of Sun Yat-sen University.Patients were divided into survival group and death group.Clinical data before operation were collected and analyzed by univariate and multivariate analysis,respectively.Risk factors influencing the perioperative mortality were selected and were used to direct the operation time.Results There were 50 patients who underwent liver transplantation and 11 cases died during perioperative phase.Five patients died of respiratory infection,three of acute renal failure,one of digestive tract hemorrhage,one of primary allograft failure and one of multiple organ failure.The perioperative,one-year and three-year survival rate were 78%,74% and 72% respectively.In the univariate analysis,age,bilirubin level,prothrombin time,creatinine level,grade of encephalopathy and MELD score were found to be significantly different between survival group and death group.The multivariate analysis revealed that age≥65 y,INR ≥3.5,creatinine level ≥ 2.5 mg/dl and MELD score ≥ 40 were independent risk factors affecting survival.Conclusions Liver transplantation is an effective treatment of choice for acute liver failure.Appropriate timing of transplantation helps achieve a high survival rate.

ABSTRACT

To clarify the function of miR-103 in the differentiation of porcine preadipocyte, we carried out real-time PCR to detect the expression pattern of miR-103 during adipogenesis, and clarified its expression tendency through cell differentiation. Then we used adenovirus that overexpressed miR-103 to infect porcine preadipocyte. Subsequently, mRNA and protein expression of adipogenesis marker--PPARgamma and aP2 was analyzed by real-time PCR and Western blotting. At last, Oil-Red O staining was used to detect lipids accumulation in the 8th day after adipogenic inducement. The expression of miR-103 increased during adipocyte differentiation; compared with the control, the preadipocyte infected by pAd-miR-103 had an elevated expression level of adipocyte marker gene PPARgamma, aP2, and obvious lipid droplet was seen in the 8th day after adipogenic inducement. These results showed that miR-103 can enhance adipogenesis in primary cultured porcine adipocytes.
